Back from his leave of absence, Doc finds the department profoundly changed: Giulia has become the head of the unit, and staff cuts are putting everyone under pressure. As Andrea tries to rally the team, a potential error threatens to have serious consequences for a patient.

The atmosphere in the ward is becoming increasingly tense: Martina faces hostility from Riccardo, while Matteo struggles to fit in with the team. While awaiting the outcome of an investigation into a serious error, Doc receives a mysterious anonymous letter that appears to hide a disturbing truth.

New revelations about Campigli reignite the conflict with Andrea, threatening to jeopardize a major project of Giulia's. Meanwhile, Riccardo becomes the face of a campaign supporting the National Health Service, but his newfound popularity complicates his relationship with a patient.

With Giulia and Campigli away from the Policlinico, Doc attempts to restore the department to its previous organizational structure. However, new admissions increase the workload and put the team under severe strain, fueling tension and discontent - and Martina risks paying the price.

Giulia and Damiano are handling the delicate case of a patient awaiting a transplant, while the arrival of Claudia brings Cesconi's past back to the surface. Meanwhile, Andrea is trying to recover, but he must rely on the care of Lidia - a doctor with whom his relationship is anything but simple.

Federico's return shakes up the department and puts Lin and Martina in a difficult position. While Doc seeks a solution to help the young man, Giulia and Campigli fear that his involvement could bring a long-guarded secret back to light.

Riccardo is looking into the case of a mysterious young woman, while jealousy and tension complicate his relationship with Martina and Matteo. Meanwhile, Damiano is anxious about his nephew Pietro's health, and Andrea is eager to finally return to the hospital wards.

Andrea continues to look into Campigli's past, and a new hospital admission might provide him with the chance to uncover what the Medical Director is hiding. Meanwhile, Riccardo is treating a patient who, much like himself, is regarded by everyone as a hero.

Giulia's mistake threatens to have irreparable consequences for everyone in the department. While she confides in Andrea, who urges her to tell the truth, Claudia is also torn over whether to tell Damiano what she witnessed.

Giulia does her best to support Andrea's decision, but he immediately chooses to bypass her and orders the hospitalization of a young woman with "diving suit syndrome" - a condition for which there seems to be nothing more that can be done.
